POSITION PURPOSE (Summary)
The Dialysis Registered Nurse is a self-motivated professional registered nurse and resource person, who assesses, plans, implements and evaluates care for the patients in the Dialysis Unit. She / he has knowledge relative to the ages served.
MINIMUM REQUIREMENTS
(Education, License / Registration, Experience, Knowledge and Abilities)
- Graduate of an accredited school of nursing. Registered nurse currently licensed to practice professional nursing in the State of Montana.
- Minimum of one (1) year MedSurg experience.
- Completion of ACLS Course within one (1) year of employment.
- Must maintain and provide documentation of professional development requirements including BLS.
OSHA CATEGORY CLASSIFICATION
Category classifications have been developed to assist employees in determining their risk for occupational exposure to bloodborne pathogens. These categories are:
Category I Employee will have occupational exposure to bloodborne pathogens in the performance of their routine jobs.
Category II Employee may have occupational exposure to bloodborne pathogens in the performance of their routine jobs.
Category III Employee will not have exposure to bloodborne pathogens in the performance of their routine job.
GENERAL RESPONSIBILITIES
Scope of practice includes:
- Utilizing assessment, diagnosis, planning, intervention, patient teaching and evaluation to provide age specific direct care for assigned patients.
- Guiding and supervising other personnel regarding delegated care.
ESSENTIAL JOB FUNCTIONS
- Delegates patient care to team Dialysis RNs as needed.
- Provides appropriate diet for patients according to medical condition and diagnostic indicators.
- Assesses patients for signs / symptoms of abuse or neglect.
- Takes and records vital signs.
- Evaluates for age appropriate behavior, motor skills and physiologic norms.
- Calculates medication doses for patient’s medical condition and diagnostic indicators.
- Administers medication doses for patient’s medical condition and diagnostic indicators.
- assesses biophysical aspects including bowel and bladder habits and nutritional needs.
- Performs ongoing assessment of skin integrity and assists patients with position change, as needed.
- Assesses patient’s mental, physical and emotional status on a daily basis.
- Prepares patient, family and caregivers for discharge plan.
- Assesses physical mobility and activities of daily living of patients.
- Assesses psycho-social aspects of patient.
- Demonstrates competency in Meditech computer applications for documentation purposes.
- Carries out appropriate isolation procedures.
- Obtains specimens such as urine, sputum, stool and wounds for various cultures.
- Clarifies and implements physician orders.
- Orients patient to the health care environment and their medical status.
- Monitors operational functions of equipment and gauges within the water treatment room.
- Tests levels of chlorine, TDS, resistivity and water hardness.
- Mixes bicarbonate and acid for each patient’s prescribed dialysis.
- Communicates pertinent patient data to health care team members.
- Administers IV fluid and medications.
- Performs CPR when needed.
- Demonstrates competency in the use of medical equipment for patient care (hemodialysis machines, pulse oximeter, glucoscan, IV pump, and crash carts).
- Accurately and timely documents patient care reports as directed by policy.
- Accurately and timely completes verbal shift reports on patient’s status.
- Assists in providing direct patient care as needed to meet all patient needs.
- Provides medical treatments to patients as ordered by physician.
INFREQUENT / MARGINAL JOB FUNCTIONS
- Acts as a preceptor to newly hired staff members.
- Answers the telephone and takes appropriate messages.
- Participates in nursing, hospital committees or CQI teams.
