First, a health care plan is developed under the direction of your personal physician.  Using this plan, a trained team of Home Health professionals coordinates the medical and social services you need.  They work with you, your family, and physician to provide the finest care in the most cost effective way.  Each plan is flexible, changing as your health needs change.

Home Nursing Care – special attention by skilled registered nurses training in Home Health Care.

Physical Therapy – therapeutic evaluation, restoration and maintenance programs to regain previous level of function.

Speech Therapy – restoration of language functions affected by recent illness or injury.

Occupational Therapy – evaluation, treatment, and consultation to enhance adaptation and functional performance of activities of daily living.

Human Services

Medical Social Counseling – help in copying with illness and related lifestyle changes.

Homemaker & Home Health Care Aides – assist with personal care and rehabilitation, monitor condition, and assist with household responsibilities.

Supplies – dressing, catheter trays, feeding tubes, etc.
